Job summary

Directions for Living in Largo, FL seeks an Intensive Case Manager - FITT to provide case management in a home-, school-, and community-based wraparound program for families with young children.

You will collaborate with an IDT, develop individualized, strength-based treatment plans, address trauma-informed care, and help improve housing stability while coordinating with community partners.

Qualifications

• Bachelor's degree in Human Services from an accredited college.
• Targeted Case Management Certification required.
• Experience with adults and children with serious persistent mental illness.
• Excellent writing, communication, computer, organizational and time management skills.

Responsibilities

• Provide case management to adults and families in the FITT program.
• Coordinate services in home, school, and community.
• Develop individualized, strength-based treatment plans with the IDT.
• Report progress, barriers, and safety concerns at IDT meetings.
