Job summary

Family Support Services in Largo, FL is seeking an IPT Facilitation Coordinator to lead, monitor, and document the Integrated Practice Team process for families in need. You will facilitate meetings, route cases to resources, and coordinate with CPI’s and case managers to ensure wraparound services that keep children safe at home.

Responsibilities include maintaining IPT records, attending referral reviews, auditing programs, and collaborating with community agencies to support family

Qualifications

• Bachelor’s Degree required; facilitation/mediation certification preferred.
• Three years’ experience with at-risk children/families; drivers license required.

Responsibilities

• Lead the Integrated Practice Team (IPT) by facilitating a core group of representatives from differing organizations whose purpose is to arrange wrap around and strength based services to allow children to remain safely in their homes
• Maintain IPT records on incoming correspondence, action documents, schedule, and follow up on each case; enter notes and action plans into FSFN
• Attend Family Preservation Team referral review meetings; network with community agencies to support IPT and Family Preservation
• Assist with referrals for programs such as FAST, STEPS, Safety Management, and High-Risk Newborn
• Perform audits of Family Preservation Programs
• Participate in special projects and other departmental requirements
