United Future is seeking a full-time Internal Medicine Physician to join our outpatient clinic in Illinois. This in-person role focuses on preventive care, chronic disease management, and community health within a multidisciplinary team.

You will work Monday–Friday, daytime schedule, and collaborate with primary care providers, nurses, medical assistants, and support staff to deliver high-quality, patient-centered care. MD/DO and active state license are required.

Qualifications

• MD or DO from an accredited medical school.
• Board Eligible or Board Certified in Internal Medicine.
• Active state medical license (or ability to obtain).
• Experience in outpatient or community-based care preferred.
• Strong clinical judgment, communication skills, and team-oriented approach.
• Commitment to providing high-quality care to underserved or diverse populations.

Responsibilities

• Provide comprehensive outpatient internal medicine care to adult patients (in Ambulatory/outpatient setting).
• Diagnose, treat, and manage acute and chronic medical conditions.
• Deliver preventive care, health screenings, immunizations, and wellness services.
• Develop and manage individualized treatment plans and follow-up care.
• Collaborate with interdisciplinary clinical teams and coordinate specialty referrals.
• Deliver culturally competent care with consideration of social determinants of health.
• Participate in quality improvement initiatives, care coordination, and population health efforts.
• Accurately document patient encounters in the electronic health record (EHR).
• Engage in clinical meetings, care team collaboration, and community health initiatives.
