Qualifications

• MD or DO from an accredited medical school.
• Board Eligible or Board Certified in Internal Medicine.
• Active state medical license (or ability to obtain).
• Experience in outpatient or community-based care preferred.

Responsibilities

• Provide comprehensive outpatient internal medicine care to adult patients.
• Diagnose, treat, and manage acute and chronic conditions.
• Deliver preventive care, health screenings, immunizations, and wellness services.
• Develop and manage individualized treatment plans and follow-up care.
• Collaborate with interdisciplinary clinical teams and coordinate referrals.
• Deliver culturally competent care with consideration of social determinants of health.
• Participate in quality improvement initiatives, care coordination, and population health.
• Accurately document patient encounters in the EHR.
• Engage in clinical meetings and community health initiatives.

Physicians work closely with an integrated care team that may include primary care providers, specialists, advanced practice providers, nurses, medical assistants, and support staff.
