LRSolutions, LLC is seeking a full-time Internal Medicine Physician (MD/DO) for an in-person outpatient role in Illinois. The position offers a daytime schedule, Monday–Friday, with competitive compensation and a focus on prevention and chronic disease management within a mission-driven, multidisciplinary team.

You will provide comprehensive outpatient care to adult patients, collaborate with a diverse care team, and participate in quality improvement and community health initiatives.

Qualifications

• MD or DO from an accredited medical school.
• Board Eligible or Board Certified in Internal Medicine.
• Active state medical license (or ability to obtain).
• Experience in outpatient or community-based care preferred.
• Strong clinical judgment and communication skills.
• Commitment to underserved or diverse populations.

Responsibilities

• Provide comprehensive outpatient internal medicine care to adult patients.
• Diagnose, treat, and manage acute and chronic medical conditions.
• Deliver preventive care, health screenings, immunizations, and wellness services.
• Develop and manage individualized treatment plans and follow-up care.
• Collaborate with interdisciplinary clinical teams and coordinate specialty referrals.
• Deliver culturally competent care with consideration of social determinants of health.
• Participate in quality improvement initiatives, care coordination, and population health efforts.
• Accurately document patient encounters in the electronic health record (EHR).
• Engage in clinical meetings, care team collaboration, and community health initiatives.
