Qualifications

• Graduate NP program
• Bachelor's and Master's degrees
• Current NY NP certification

Responsibilities

• Introduces/identifies self as a nurse practitioner
• Assists the physician with patient management focusing on quality care and timely discharge
• Communicates professionally with physicians, case managers, nurses and hospital staff
• Performs initial and ongoing assessment/history and physical examination
• Orders and interprets tests and results as needed
• Documents care in patient chart following CDI and hospital policies
• Writes legibly and dates all chart entries
• Assists in surgery and perioperative care as required
• Documents discharge instructions and ensures discharge planning is complete
