Job summary

NYU Langone Hospital—Long Island in East Meadow, NY, is seeking a Nurse Practitioner – Internal Medicine to provide patient-centered care across the care continuum. The role emphasizes assessment, planning, implementation and evaluation with collaboration from a multidisciplinary team.

The position includes patient and family-focused planning, in-patient and discharge prescriptions under NYS law, and participation in professional nursing standards and education.

Qualifications

• Master's Degree graduate of an accredited Nurse Practitioner program.
• Current NYS Registered Nurse License.
• Current NYS Nurse Practitioner License.
• Current Nurse Practitioner Certification.
• Excellent communication skills (oral & written).
• Basic computer skills.

Responsibilities

• Performs other duties as assigned.
• Plans and organizes care to meet individual patient needs and to ensure appropriate clinical resource utilization per protocols, pathways, and other means.
• Works collaboratively with the interdisciplinary team to develop and implement the therapeutic plan of care for a group of patients and ensure communication with family members.
• Participates in developing and revising nursing standards and patient education materials.
• Maintains timely communications with family members, house staff and other disciplines.
• Ensures timely communications with the nursing staff and leadership.
• Provides consultation about the therapeutic regimen to nursing staff.
• Facilitates continuity of care across the continuum of services (home care, other facilities, appointments).
• Demonstrates knowledge of Services Standards and incorporates them into responsibilities.
• Participates in activities related to professional nursing standards and regulations.
• Functions as a respectful member of the healthcare team.
• Performs complete health history, physical exam and psychosocial assessment.
• Orders laboratory, radiological and diagnostic studies per protocols and follows up on results.
• Formulates plan of care with attending physician, patient and family.
• Identifies and implements plans that maximize patient/family preferences and outcomes.
• Prescribes medications in-patient and at discharge per NYS law (including controlled substances).
• Prescribes medical equipment and home health services per protocol.
• Practices as a member of the allied health staff per bylaws.
• Evaluates treatment and adjusts plans per standards.
