Job summary

Responsibilities

• Manage assigned panels and coordinate care within the primary care/outpatient setting.
• Provide health promotion, disease prevention, and chronic care management.
• Deliver and improve patient-centered care through collaboration with the interdisciplinary team.
• Utilize nursing processes to assess, diagnose, plan, implement, and evaluate patient care.
