Qualifications

• Foreign nursing education requires degree equivalency validation

Responsibilities

• Provide gender-specific direct Veteran care and care coordination in Primary Care
• Manage assigned panels and ensure appropriate evaluation and access
• Promote health education, wellness coaching, and chronic care management
• Develop care plans based on nursing assessments and patient needs
• Demonstrate leadership to improve care within the Primary Care Department
