Qualifications

• English language proficiency required for direct patient care.

Responsibilities

• Provide competent, evidence-based primary care to Veterans.
• Supervise licensed practical/vocational nurses and nursing assistants as applicable.
• Manage assigned patient panels and utilize registries for care access.
• Deliver health promotion, disease prevention, and chronic care management.
• Collaborate with interdisciplinary team to improve patient outcomes.
• Participate in maintaining safe, high-quality outpatient nursing care.
