Interim HealthCare of St. Louis is hiring a Community Home Care Coordinator for a salaried, full-time role based mainly at an Independent Living Community. You will coordinate client care, welcome new clients, support caregivers, and maintain care plans across our service area with field and office collaboration.

Qualifications

• Experience in home care, senior living, caregiving, or care coordination.
• Experience guiding caregivers and conducting client visits preferred.
• Strong communication and organizational skills.
• Ability to travel throughout service area.
• Commitment to client confidentiality and safety.

Responsibilities

• Coordinate client care and update care plans with the office team.
• Welcome new clients and complete enrollment paperwork.
• Support caregivers with check-ins and attendance.
• Meet caregivers at client homes to review care plans.
• Maintain AxisCare documentation and attend weekly meetings.
• Assist with staffing and coverage across the service area.
• Foster relationships with residents, families and staff.
