Job summary

Randstad Enterprise is seeking a dedicated Case Manager for the Intensive Mobile Treatment (IMT) program in New York City. You will provide person-centered, community-based support across shelters, hospitals, and community settings, delivering crisis intervention, recovery planning, and coordination with healthcare and criminal justice partners.

Qualifications

• Bachelor’s degree in social work or related field required.
• Experience with mental health, homelessness, or criminal justice settings preferred.
• Strong communication, organization, and problem-solving skills.
• Ability to work independently and in a multidisciplinary team.
• Proficient with MS Office and electronic documentation.

Responsibilities

• Provide case management, counseling, recovery planning, and crisis intervention.
• Develop individualized service plans and support clients toward recovery goals.
• Assist with housing applications, placement, employment, and public benefits.
• Conduct outreach and accompany clients to appointments across NYC.
• Advocate for clients and coordinate with healthcare, shelters, and justice agencies.
• Maintain accurate case records and required documentation.
• Manage caseload with prioritization of urgent needs and ongoing engagement.
• Participate in interdisciplinary team meetings to ensure continuity of care.
