Job summary

Bond Health is seeking a Registered Nurse for the Intensive Mobile Treatment Team (IMT) in Brooklyn, NY. The RN will assess and monitor participants' clinical health, coordinate care plans with a multidisciplinary team, and deliver care directly in community settings such as shelters and outreach sites.

The role requires NY RN license, minimum two years’ nursing experience, experience with mental health, strong communication, and ability to travel within the community.

Qualifications

• Licensed as an RN by the New York State Education Department (NYSED).
• Minimum 2 years of experience as a Registered Nurse.
• Minimum 2 years of experience with individuals with mental health conditions in health or social services settings.
• Strong written, verbal, and interpersonal communication skills.
• Eligible for full Medicaid/Medicare participation.
• Proficient in MS Word, Excel, PowerPoint; experience with electronic health systems.

Responsibilities

• Conduct psychiatric and physical health assessments for program participants.
• Develop and coordinate care plans with the multidisciplinary treatment team.
• Monitor participant health status and modify treatment plans as needed.
• Administer medications, injections, vaccines, and perform phlebotomy as ordered.
• Monitor vital signs and communication of findings to the treatment team.
• Coordinate medical appointments and collaborate with psychiatrists and community physicians.
• Provide treatment, rehabilitation, and recovery support services.
• Advocate for participants and assist with accessing community resources and services.
• Maintain accurate clinical documentation, progress notes, and medical records.
• Participate in team meetings, supervision, and required trainings.
• Provide crisis intervention services on a rotating basis.
