Blue Kite Wellness is seeking an Intensive Home-Based Clinician II (IHBC) to support children, youth, and families in clients’ homes and communities, with telehealth when permitted. This field-based hybrid role emphasizes assessment-informed care and family-centered, strength-based practice.

Qualifications include QMHP or LPHA, a Master’s degree in a related field, and at least two years of relevant experience.

Qualifikationen

• QMHP or LPHA per IL requirements.
• Master’s degree in social work, counseling, psychology, or related field.
• At least two years of behavioral health experience with children, adolescents, and families.
• Strong assessment, engagement, treatment-planning, and care-coordination skills.
• Ability to work independently in home and community settings while collaborating with multidisciplinary team.

Aufgaben

• Provide intensive home-based clinical services to eligible children, youth and families.
• Develop and adjust individualized clinical interventions using IM+CANS and other assessments.
• Deliver individual, family, caregiver, and cross-system therapy to improve functioning and safety.
• Strengthen caregiver capacity and family communication.
• Provide clinical direction to IHBS staff and collaborate with Care Coordinators and schools.
• Participate in supervision, case consultation, and ongoing training.
