Blue Kite Wellness seeks an Intensive Home-Based Clinician II to support children and families in Arlington Heights and surrounding areas. The role delivers assessment-informed, short-term behavioral health services in home and community settings with telehealth where permitted.

You will develop and implement individualized intervention plans, collaborate with youth, caregivers, and Care Coordinators, and supervise IHBS staff as needed.

Formación

• Meets current Illinois requirements for a QMHP or LPHA, including education, licensure, supervision, credentialing, and scope.
• Master’s degree in social work, counseling, psychology, or related behavioral health field from an accredited institution.
• At least two years of progressively responsible behavioral health experience with children, adolescents, and families.
• Strong clinical assessment, engagement, treatment planning, family intervention, crisis prevention, de-escalation, documentation, and care coordination.
• Ability to work independently in home and community settings while collaborating with a multidisciplinary team.

Responsabilidades

• Provide Intensive Home-Based Clinical services to eligible children, youth, and families per approved service plans.
• Use IM+CANS, Functional Behavioral Assessment, clinical observations, family input, and team recommendations to guide interventions.
• Deliver individual, family, caregiver, and cross-system therapies to reduce symptoms and improve family functioning.
• Strengthen caregiver capacity, family communication, problem-solving, coping, and emotional regulation.
• Provide clinical direction and consultation to IHBS staff and review progress with the team.
• Participate on the Child and Family Team and collaborate with caregivers, schools, and other providers.
• Support crisis prevention, safety planning, de-escalation, and post-crisis follow-up within scope.
• Monitor progress, identify needs or safety concerns, and adjust services accordingly.
• Complete timely documentation per Blue Kite Wellness, payer, and Illinois HFS requirements.
• Engage in supervision, case consultations, meetings, trainings, and ongoing professional development.
