Job summary

Amity Foundation in Los Angeles seeks compassionate individuals to join as Intensive Case Manager, helping people experiencing homelessness and those re-entering after incarceration by coordinating housing, mental health, substance use treatment, and community resources.

Responsibilities include interviewing participants, welfare checks, developing treatment plans, linking to benefits, coordinating with housing staff and probation, maintaining records, and transporting clients as needed.

Qualifications

• One year of experience with PEH and a BA in social work, psychology or related field, OR two years of case management for similar populations.
• Knowledge of assessments, counseling, reentry services, and culturally diverse populations.
• Excellent organizational and communication skills; familiarity with non-profit settings.

Responsibilities

• Interview and assess participant needs through referrals.
• Conduct wellness checks and provide crisis management.
• Develop treatment plans to address services and goals.
• Assist with public benefits advocacy and linkage.
• Coordinate housing, mental health, and substance abuse resources.
• Collaborate with housing staff, probation officers, psychiatrists, and other partners.
• Maintain concise case records and documentation according to funding guidelines.
• Use company or personal vehicle to transport clients to appointments or visits.
