Alcott in Culver City, CA, seeks a Case Manager – ODR (ICMS) to join our Intensive Case Management Services team. You will perform outreach, housing navigation, and service coordination for adults experiencing homelessness, helping secure permanent housing and health services across Los Angeles County.

The role is field-based and on-site with a 8:30 a.m.–5:00 p.m. schedule. A bachelor’s degree in a related field and relevant experience are required.

Qualificações

• At least one year of experience working with individuals experiencing homelessness and a related bachelor’s degree or equivalent experience.
• Experience with individuals with mental illness, chronic medical conditions, and substance use disorders.
• Knowledge of homelessness, outreach strategies, case management models, crisis intervention, and eviction prevention.
• Ability to perform required computer functions for electronic health record documentation.

Responsabilidades

• Conduct outreach in collaboration with DHS and other partners to individuals referred from the jail system.
• Engage participants to determine eligibility for permanent supportive housing and coordinate housing applications.
• Develop and update individualized service plans with quarterly reviews and updates.
• Provide crisis intervention and ongoing tenancy support as needed; document services timely in the EHR.
