Job summary

WestCare Foundation in Fresno, CA is seeking a Case Manager to coordinate housing-focused reentry services for adults. You will provide intake, assessment, and ongoing case management for up to 25 participants, guiding them through housing interventions and connecting with health, employment, and supportive services.

Responsibilities include maintaining client files and data, reporting, and collaboration with Housing Locator and community partners.

Qualifications

Minimum two years’ experience in social services, case management, or related human services field.
Strong knowledge of housing programs, supportive services, and community resources.
Excellent communication, public speaking, and interpersonal skills.
Strong computer skills, including Microsoft Office (Word, Excel, PowerPoint).
Good documentation and organizational skills.
Ability to work independently, manage multiple priorities, and remain calm.

Responsibilities

Provide case management services to a caseload of participants, up to 25 at a time.
Navigate clients through Coordinated Entry System to housing intervention.
Provide Rapid Rehousing supportive services and promote housing stability.
Maintain communication with Bridges to Success Housing Locator and team.
Intake and assessment to determine eligibility and services.
Maintain client files, develop service plans, and track data.
Make referrals to agencies and assist with paperwork.
