Job summary

Project 180 is seeking a Project Based Housing Case Manager to deliver intensive wraparound case management on-site and in-field within Los Angeles. You will engage justice-involved clients, coordinate with a multidisciplinary team, and support recovery goals using a harm-reduction framework.

The ideal candidate has a bachelor’s degree and at least 1 year of case management experience, with strong communication and cultural competence to work with diverse populations.

Qualifications

• Bachelor’s degree in any field and at least 1 year of case management experience.
• Experience working with culturally diverse populations.
• Valid CA driver’s license, proof of insurance and own vehicle.
• Must not be on any type of legal supervision (probation or parole).
• Verification of Employment Eligibility and Background Check.
• Basic computer skills.

Responsibilities

• Provide intensive wraparound service on site and in the field.
• Engage clients with strong rapport and motivational interviewing.
• Coordinate care with a multidisciplinary treatment team.
• Develop knowledge of community resources and connect clients to services.
• Assist with court appearances and coordination with agencies.
• Maintain case notes and program data as required.
