Job summary

Amity Foundation in Los Angeles is seeking compassionate individuals for the Case Manager(Advocate) role. This position provides direct casework within STOP programs, including intake, needs assessments, CMPs, and progress monitoring.

You will develop CMPs within 45 days, conduct at least two face-to-face sessions monthly, maintain meticulous records, and coordinate referrals with partner agencies. The role emphasizes advocacy, confidentiality, and collaboration with program leadership.

Responsibilities

• Provide interactive services to STOP participants.
• Develop Case Management Plans (CMP) for participants.
• Monitor progress relative to CMP through all program phases.
• Make appropriate referrals to outside agencies.
• Maintain progress notes in participant files.
• Keep the STOP Program Director and liaisons informed of progress.
• Maintain comprehensive case records and statistics for STOP participants.
• Develop CMP within 45 days of placement with goals based on assessments.
• Provide at least two face-to-face case management sessions per month.
• Attend workshops, meetings, and trainings as requested.
• May serve as Community Advocate as needed.
• Perform other related duties as assigned.
