Job summary

Linc Housing Corporation is seeking a Case Manager for its Intensive Case Management Services in Inglewood. You will coordinate case management for formerly homeless individuals with chronic conditions, using motivational interviewing and harm-reduction approaches.

You’ll collaborate with property managers, VA staff, and service providers to prevent housing loss and support client goals, with strict HIPAA compliance and strong documentation practices.

Qualifications

• Experience in case management for homeless or vulnerable populations.
• Ability to coordinate services with multiple partners and agencies.
• Proven crisis intervention and service coordination capabilities.

Responsibilities

• Work with referring agencies such as the VA and Housing Authorities.
• Assist with lease-up paperwork.
• Coordinate services to help residents achieve treatment plan goals.
• Develop rapport and connect residents to mental health, housing, and support services.
• Document time, mileage, and program data accurately and timely.
