Job summary

Linc Housing is seeking an Intensive Case Management Program Case Manager in Inglewood, CA. This full-time role focuses on coordinating services for formerly homeless individuals with chronic medical conditions and co-occurring disorders, using motivational interviewing and evidence-based approaches.

You will work with families and a network of partners, including housing locators and property management, to prevent housing loss by enhancing problem solving and self-care.

Qualifications

• Two years’ experience working with homeless individuals or vulnerable populations.
• Experience with mental health/substance use services is a plus.
• Strong writing and interpersonal skills required.

Responsibilities

• Coordinate services using motivational interviewing and harm reduction.
• Assist with paperwork for lease-up processes for residents.
• Collaborate with Linc staff, VA staff, and property managers.
• Develop connections to mental health, housing, and employment services.
• Provide case management and service coordination to reach treatment plan goals.
• Maintain timely and accurate documentation and data reports.
