Job summary

Linc Housing Corporation is seeking an Intensive Case Manager in Willowbrook to join the Intensive Case Management Services team. You will coordinate services for formerly homeless individuals with chronic conditions, using motivational interviewing, harm reduction and trauma-informed care.

Reports to the Senior Case Manager of ICMS and collaborates with partners. The role requires strong assessment, planning and coordination skills, with a focus on preventing housing loss and supporting

Qualifications

• Two years’ experience working with homeless individuals, or similar vulnerable populations.
• Strong written and verbal communication skills.
• Ability to assess and coordinate services using evidence-based practices.
• Knowledge of motivational interviewing, harm reduction, and trauma-informed care.

Responsibilities

• Coordinate services using evidence-based case management practices.
• Collaborate with VA, housing authorities, and housing locators.
• Develop and maintain rapport with residents and community partners to connect services.
• Provide program coverage and support groups as needed.
• Maintain accurate documentation and timely reporting.
