Job summary

DESC, based in Seattle, seeks a Mental Health Case Manager for HOST to serve adults facing homelessness and severe mental illness. The role requires outreach, intake, service planning, and coordinating housing, medical care, and benefits to promote stability and transition to long‑term services.

You’ll collaborate with a multidisciplinary team and partner agencies, advocating for clients while prioritizing crisis intervention and continuity of care.

Qualifications

• Minimum bachelor’s degree in social work, psychology, or related field.
• Ability to meet WA licensing requirements for AAC or equivalent credential.
• Interest in engaging difficult clients and applying recovery-focused approaches.
• Knowledge of harm reduction methods and community resources.
• Bilingual English/Spanish is a plus.

Responsibilities

• Provide outreach and engagement services in office, streets, shelters, encampments, and community areas.
• Conduct assessments, develop service plans, and link clients to housing, healthcare, and benefits.
• Coordinate all aspects of clients' support and treatment plans, including housing access and finances.
• Advocate for client access to community resources and ensure continuity of care.
• Prioritize crisis intervention and coordinate with emergency and crisis services as needed.
• Collaborate with DESC staff and external partners for full continuity of care.
