Job summary

Providence St. Peter Hospital in Olympia seeks an Intensive Case Manager to serve outpatient psychiatric clients, collaborating with the Community Care Center and home health team.

This role provides short-term intensive care coordination, housing resources, and health care referrals, supports stabilization, and extends care after transitions to adult services. It also coordinates group therapy and conducts clinical assessments, crisis intervention, and care transitions.

Qualifications

• Master's degree in Mental Health counseling or equivalent required.
• Two years of acute care or community-based mental health work preferred.

Responsibilities

• Provide services to outpatient psychiatric clients and coordinate care with partners.
• Identify vulnerable community members and link to housing resources and healthcare providers.
• Deliver short-term intensive care coordination and ongoing stabilization in the community.
• Coordinate group psychotherapy and perform clinical assessments, crisis intervention, and referrals.
• Support clients' successful transition to adult case management services when appropriate.
