Job summary

DESC’s SAGE intake and case management team in Seattle supports adults with severe mental illness to achieve recovery and greater independence. You’ll work within a Recovery model, coordinating psychiatric and social services, assisting with housing access, and collaborating with community providers.

Positions require a relevant bachelor’s degree or equivalent experience, readiness to travel for outreach, and a commitment to compassionate, stigma‑free care.

Qualifications

  • Relevant bachelor’s degree in social work, psychology, or related behavioral science, OR
  • A combination of 1 year of relevant paid work experience and demonstration of the ability to perform required job duties.
  • Ability to drive a vehicle and WA driver’s license.

Responsibilities

  • Participate as a member of a multi‑disciplinary team providing on‑going case management services to adults with severe and persistent mental illnesses.
  • Provide outreach and engagement services to assigned clients to engage and maintain relationships.
  • Conduct psychosocial assessments; develop goal planners with consumer participation; facilitate linkages to resources.
