Job summary

CN Guidance & Counseling Services, Inc. seeks a dedicated clinician to join the FACT Team, delivering integrated clinical treatment, counseling, housing, and support services to individuals with severe mental illness.

The role emphasizes coordination across disciplines and community-based care. Responsibilities include assessments, case management, crisis intervention, and discharge planning, with a 35-hour workweek and occasional pager coverage.

Qualifications

• Bachelor's degree in social work, psychology, rehabilitation counseling, or related field.
• NYS licensure in a specific field is preferred.
• Two years' experience working with the chronically mentally ill (Masters level) recommended; three years' experience for Bachelor's level.
• Competency in substance abuse, family support, or vocational rehabilitation with at least one year of training/experience.

Responsibilities

• Perform routine team duties and lead integration of dual-recovery treatment with other team tasks.
• Provide Assessment and Service Planning, QA/QI processes, Utilization Review and coordination with Forensic ACT.
• Engage in assessments, case management, crisis intervention, and discharge planning.
• Maintain case records and complete documentation on time in compliance with regulations.
• Provide direct, Telehealth, face-to-face and/or collateral services in office or community settings.
• Work a 35-hour week with pager coverage as needed.
• Assist recipients in understanding and applying treatment and discharge planning.
