Job summary

CN Guidance & Counseling Services is seeking a FACT Team case manager to deliver community-based housing support and care coordination for individuals with severe mental illness. The role requires a Bachelor's degree in social work, psychology, rehabilitation counseling or related field, plus 1 year of advanced mental health experience or 2 years at the bachelor's level.

You will coordinate benefits, referrals, and ensure quality improvement efforts.

Qualifications

• Bachelor's Degree in social work, psychology, rehabilitation counseling or related field required.
• One year experience in a mental health setting at the Masters or higher level or two years' experience at the Bachelor's level.

Responsibilities

• Assist FACT Team recipients in obtaining and maintaining adequate housing.
• Must have an understanding of concrete services i.e. Medicaid, Medicare, DSS and SPOA housing.
• Completes referrals and linkage to community benefits and organizations to assist with treatment plan goals.
• Responsible for Quality Improvement, Utilization Review and Performance Improvement activities including data collection and analysis of outcomes.
• Plan and coordinate all services to support case management and care coordination, including benefits applications.
