Easterseals MORC is hiring an Integrated Healthcare Specialist in Southfield, MI, offering a hybrid work arrangement. The role focuses on coordinating care, addressing client inquiries, and supporting team-based care in an integrated healthcare setting.

The position emphasizes clerical proficiency, decision-making within policy, and collaboration with care teams to improve population health outcomes. A High School diploma or equivalent is required.

Qualifications

• High School Diploma or equivalent.
• Three years of responsible full time clerical experience.
• Ability to make decisions in accordance with departmental policies and procedures.

Responsibilities

• Assist individuals serviced by addressing inquiries, resolving issues, and fulfilling requests across diverse communication channels.
• Answering and triaging internal and external telephone calls
• Pulling, filing, autiding, and maintainingi charts for the teams assigned.
• Closing team charts
• Co-facilitating team meetings minutes including huddles regarding team-based care, integrated healthcare, and other population health
• Review and share data reports and outcomes of team services
• Assist wtih the coordination of care for individuals served on the team
• Comelting vitals and documenting in E.H.R.
