Easterseals MORC is hiring for a Community Health Worker - Family Services to help make a difference and become part of something bigger than yourself!

We are looking for Game Changers who wake up excited to make a difference and who are the superheroes of their field serving the people we support. This full-time, hybrid role is based in Walled Lake, MI, and requires a valid Michigan driver’s license with insurance.

Qualifications

• High School diploma or equivalent.
• Bachelor’s degree a plus.

Responsibilities

• Establishes trusting relationships with persons served and works with clinical care teams.
• Provides ongoing interviewing, goal setting and follow-up for persons served.
• Communicates Easterseals MORC’s mission to staff and clients.
• Acts as advocate between persons served and community service agencies.
• Assists with overcoming barriers to medical care and behavioral health services.
• Connects clients with transportation resources when appropriate.
• Assists with enrollment, eligibility and follow-up documentation.

Qualifications:

• High School diploma or equivalent. Bachelor’s degree a plus.
• At least two (2) years of experience working in a healthcare or clinical setting in the support of persons served.
• Must possess a valid driver’s license in the State of Michigan and proof of insurance.
• Ability to work closely and communicate effectively with persons with mental and/or physical limitations.

Duties and Responsibilities:

• Establishes trusting relationships with persons served as they work collaboratively and effectively with clinical care teams.
• Provides ongoing interviewing, goal setting and follow-up for persons served.
• Effectively communicates Easterseals MORC’s mission in establishing relationships with medical staff and persons served.
• Acts in advocacy and liaison role between persons served and community service agencies.
• Assists persons served in overcoming barriers to obtaining needed medical care and/or behavioral health/social services.
• Connects persons served with transportation resources when appropriate.
• Assists with the completion of required information such as enrollment, eligibility determination and overall follow-up. Assist in the completion of applications and registration forms when needed in order to expedite care.

Education

Required

High School or better.

Preferred

Bachelors or better in Human Services.

Experience

2 years:

At least two (2) years of experience working in a healthcare or clinical setting in the support of persons served.
