Job summary

Cherokee Indian Hospital Authority in Cherokee, NC seeks a Qualified MH Professional to lead case management for individuals with mental health and substance use disorders, coordinating access to medical, behavioral, social, and community services.

You will conduct assessments, develop person-centered plans, link clients to services, monitor progress, and collaborate with clinical teams to ensure safe, culturally respectful care for Native American populations.

Qualifications

• Qualified Professional definition includes licensure or degree with experience in behavioral health/substance abuse.
• License/certification in behavioral health (CSAC, LCAS) preferred.
• Bachelor’s degree with 2+ years post-bachelor’s experience or Master’s degree with 1+ year post-graduate experience.

Responsibilities

• Perform case management assessments, person-centered planning, referral and linkage.
• Provide timely client contacts and follow-up.
• Collaborate with medical staff to coordinate patient care and discharge plans.
• Develop discharge plans and link to community resources for ongoing support.
• Maintain confidential records and comply with privacy policies.
