Job summary

Qualifications

• Education: High School Diploma or GED (Bachelor’s degree preferred).
• Experience: Minimum of 3 years of case management experience.
• Physical/communication: Able to work under stress; excellent written and verbal communication.

Responsibilities

• Act as liaison between therapist, care team, client, and family to coordinate the best care.
• Maintain clear communication with family, referral source, and Path Behavioral Healthcare.
• Attend court hearings as appropriate and ensure documentation and billing comply with policies and EHR standards.
• Coordinate and monitor ISP plans and advocate for client access to services; participate in team meetings and committee work.
• Deliver crisis intervention and discharge planning support, while ensuring HIPAA compliance.
