Job summary

Behavioral Health Group seeks a Clinical Care Coordinator to coordinate care across behavioral and medical providers in the District of Columbia. Qualifications

• Requires licensure as listed (Physicians, psychologists, LICSW, LGSW, APRN, RN, LISW, LPC, LMFT, LGPC).
• Strong communication and interpersonal skills.
• Ability to manage caseload and coordinate across providers.

Responsibilities

• Facilitate outreach to establish referral providers and participate in outreach events.
• Facilitate intake admissions and reassessments as scheduled.
• Maintain complete, comprehensive patient records and ensure timely chart documentation.
• Coordinate care with external providers and ensure appropriate linkages across levels of care.
• Provide crisis intervention/support and follow-up communications after discharge.
