Qualifications

• Must hold one or more licensures: Physicians, psychologists, LICSW, LGSW, APRN, RN, LISW, LPC, LMFT, LGPC.
• Experience in behavioral health care coordination and case management.
• Familiarity with regulatory requirements and record-keeping in clinical settings.

Responsibilities

• Facilitate outreach to establish referral providers and participate in outreach events
• Manage intake admissions and reassessments as scheduled
• Maintain and update patient records with timely documentation
• Coordinate care between behavioral health and external providers
• Develop and update formal treatment plans within state guidelines
• Attend weekly treatment team meetings and communicate caseload status to the supervisor
