Qualifications

• High school diploma with medical/clinical experience.
• Experience working as a patient advocate.
• Bachelor's degree in a related field preferred.

Responsibilities

• Ensures patient primary care chart is up to date with specialty consults, hospitalizations, ER visits, and chronic disease management.
• Enter patient clinical and non-clinical information into electronic registry (e.g., e-Clinical Works, CareWare, Aries, +Care).
• Maintain tracking (SFGH alerts) to Primary care team and documentation on referrals to promote team awareness and patient safety.
• Assist patients in problem solving barriers to care (interpreters, transportation, prescription assistance).
• Act as system navigator and point of contact for patients and providers.
• Advocate for patients with carriers to obtain necessary services in a timely fashion.
• Assist patients with signing up for medical coverage.
