Job summary

Health Connect America is seeking an Integrated Care Coordinator to plan, coordinate, and monitor individualized treatment plans, aligning with behavioral health goals. You will assist the Nurse Practitioner with on-site documentation and support marketing efforts for referrals.

You will engage with clients, provide care management, ensure regulatory compliance, and help maintain medical records while coordinating with the care team and community resources.

Qualifications

• Bachelor's degree required; human services preferred.
• Experience working with children and families in case management.
• Regulatory and quality standards knowledge and adherence.

Responsibilities

• Coordinate care plans and monitor progress to align with behavioral health goals.
• Assist Nurse Practitioner with clinic-related documentation and on-site facilitation.
• Support outreach for new referrals and education on our programs.
• Ensure medical record compliance and timely documentation of care activities.
• Monitor HEDIS gaps and manage clinic census and rosters.
