Job summary

Intermountain Centers in Arizona is seeking a Care Coordinator to manage member health needs using an integrated case management model. You will assess, plan, implement, and monitor services, documenting in the Electronic Health Record and coordinating with an interdisciplinary team.

Qualifications

• Bachelor’s degree or higher in a behavioral health field or related field.
• Equivalent combination of education and experience may be considered.
• Experience in behavioral health services is preferred.

Responsibilities

• Observe members, listen to concerns, and record observations.
• Document concerns in clinical records per policy.
• Monitor health and safety and report concerns per policies.
• Maintain and update member records, assessments, and treatment plans.
• Develop individual service plans with time-specific actions.
• Complete progress notes using billing codes and meet engagement targets.
• Collaborate with care teams, primary care, and families.
• Advocate for member access to resources to support plans.
• participate in supervision as required by licensing and funding sources.
• Perform other duties related to the role as assigned.

Education

Bachelor's degree in behavioral health, social work, or related field

Associate’s degree in behavioral health + 2 years experience

High school diploma + 4 years behavioral health experience
