Qualifications

• Knowledge and skills in community based behavioral health care and case management.
• Good interpersonal and communication skills.
• Agreement to use personal transportation in work capacity.
• Willingness to use reliable personal transportation in work capacity.
• Completion of Case Management training post college degree.
• Valid driver’s license; CPI and CPR/First Aid certifications.

Responsibilities

• Provide case coordination, outreach, and advocacy to individuals entering CCBHC.
• Perform initial screening and referral services to connect to needed resources.
• Develop, implement, and revise coordinated treatment plans with professional judgment.
• Maintain EMR and program records in compliance with standards.
• Coordinate with community systems and MOUs to support linkage and crisis management.
• Provide crisis management and psychosocial rehabilitative services.
• Attend meetings and participate in supervision; complete required trainings.
• Complete DSC paperwork timely and ensure follow-up with consumers.
