Job summary

Center for Life Management (CLM) is seeking a full-time Integrated Care Community Support Counselor to deliver outpatient, community-based support across home and clinical settings. The role involves collaboration with a multidisciplinary team to support individuals with co-morbid health needs.

The position offers a 35-hour work week, with opportunities for advanced training and education. CLM has partnered with New England College to offer a Master’s Degree in Clinical Mental Health Counseling

Qualifications

• Bachelor’s degree in social work, mental health counseling, psychology, or nursing.
• Ability to exercise sound clinical judgment in routine, urgent, and emergent situations.
• Knowledge of chronic co-morbid physical health conditions and experience with CFI services.
• Proficient computer skills with Microsoft Office.
• Ability to document effectively and efficiently in clinical work.
• Ability to navigate an electronic health record.

Responsibilities

• Provide counseling in the home, medical office or other community-based setting.
• Develop, apply, and reinforce coping strategies.
• Provide crisis intervention to ensure client safety.
• Provide timely documentation on each service delivered.
• Work with treatment team and partners to ensure wrap-around care.
• Set hours of billable service each week to meet KPIs.
• Represent CLM professionally in all settings.
• Perform duties as assigned by supervisor.
