MyMichigan Health is seeking a Behavioral Health Therapist to join the primary care treatment team. The therapist identifies, assesses, triages, and manages patients with medical and behavioral health problems, delivering brief evidence-based treatments and patient education to support self-management and population health.

The clinician collaborates with the patient’s medical provider, coordinates care, and supports crisis intervention and timely follow-up within an integrated care model.

Qualificações

• Experience in case management and triage within primary care settings.
• Knowledge of evidence-based behavioral health interventions and brief therapies.
• Ability to document treatment plans and outcomes in electronic health records.
• Strong communication with multidisciplinary teams and patients/families.

Responsabilidades

• Assess clinical status of patients referred by the PCP and coordinate care with the primary care team.
• Use brief assessments to triage and monitor treatment response; document outcomes.
• Assist in program development for conditions like pain, diabetes, weight loss, insomnia, stress.
• Track follow-up and outcomes in the registry and document progress in the EHR and related systems.
