MyMichigan Health is seeking a Behavioral Health Therapist to join the primary care treatment team. The BHT identifies, assesses, triages, documents and manages patients with medical and behavioral health problems, using psycho-education and brief evidence-based interventions.

The role supports mental and physical health on an assigned caseload, provides crisis intervention, and coordinates care within the primary care team to ensure timely access to services.

Qualifications

• Master’s degree in psychology or social work and MI licensure preferred.
• Licensure as Licensed Psychologist or Master’s Social Work in Michigan.
• Experience in integrated behavioral health in primary care settings preferred.
• Membership in professional organizations (APA, NASW, CFHA, MPA, SSWLHC-MI) preferred.
• Ability to work with diverse populations and crisis intervention.

Responsabilités

• Assesses clinical status of patients and intervenes to prevent deterioration; collaborates with primary care providers.
• Uses brief assessment tools to triage and measure outcomes; documents treatment response and follows-up.
• Assist in program development (pain, diabetes, weight loss, insomnia, stress) with patient education materials.
• Track follow up and outcomes using a registry; document in EHR and share with care team.
