MyMichigan Health is seeking a Behavioral Health Social Worker with an MSW to support patients on the mental health unit. The role involves assessments, discharge planning, education, counseling, and coordinating care across the continuum to improve outcomes.

Responsibilities include crisis management, care coordination, and collaboration with the healthcare team to ensure safe transitions to appropriate levels of care and reduce avoidable days.

Qualificações

• Master's degree in social work is required.
• Licensure as a Master Social Worker in Michigan or actively pursuing licensure.
• Mandatory training: Recipient Rights, CPI, TEAM ADVANCED.
• Experience in behavioral health and discharge planning is preferred.

Responsabilidades

• Complete and document psycho-social assessments to remove barriers and plan discharge.
• Provide social work interventions for crisis management, discharge planning, and transitional care.
• Co-lead daily care coordination meetings with the care team to identify high-risk factors.
• Collaborate with patient, family, and healthcare team to transition to appropriate level of care and prevent readmission.
