Job summary

Qualifications

• Masters Degree in Social Work or related field.
• Licensure in Michigan as a psychologist or Master’s Social Worker.
• Knowledge of Integrated Behavioral Health practices.

Responsibilities

• Assesses the clinical status of patients referred by the primary care provider and coordinates care with the primary care team.
• Uses brief assessment tools to triage and track treatment response and outcomes.
• Assists in program development for areas like pain, diabetes, weight loss, insomnia, stress, etc.
• Tracks patient follow-up and clinical outcomes in the EHR and required systems.
