Job summary

Great Lakes Bay Health Centers in Saginaw, MI is seeking a licensed Master’s social worker to provide behavioral health services, including assessments, counseling, and care coordination. The LMSW will collaborate with the care team to address mental health and substance use needs, and to develop plans to improve patient self-management.

Qualifications

• Skilled in written and oral communication.
• Able to demonstrate positive customer service skills.
• Basic computer skills and ability to work in an electronic medical record.

Responsibilities

• Accept referrals from care team for behavioral health needs.
• Provide assessments, diagnosis, consultation, and interventions for behavioral health and/or substance use needs.
• Assist in developing care management processes to improve self-management of chronic conditions.
• Consult with patient and care team regarding recommended behavioral health interventions.
• Assist in detection of at-risk patients and develop prevention plans.
• Maintain accurate and timely documentation in EMR.
