Missouri Delta Medical Center in Sikeston, MO is seeking a Behavioral Health Assistant to support an Integrated Primary Care Health Home Program. Qualifications

• Bachelor of Science in Social Work, Psychology, Human Services, or a closely related behavioral health field.
• Minimum of 1 year of hands-on experience within a behavioral health, mental health, or substance use care setting; primary care or health home experience valued.
• Must complete additional program-specific training provided upon hire regarding standing orders, care paths, integrated screening protocols, and brief intervention modalities.

Responsibilities

• Conduct age-appropriate preventive health screenings to identify behavioral health needs, substance use, health risk behaviors, and social determinants of health.
• Administer secondary screeners for patients with positive initial findings to determine education, coaching, or referral needs.
• Route high-risk screening results to the supervising Behavioral Health Consultant for assessment or crisis management.
• Provide patient education, health/wellness coaching, and reinforcement under standing orders and clinical protocols.
• Assist with self-management goal setting, action planning, and routine follow-up to reinforce care plan objectives.
• Provide outreach and engagement for complex patients to improve navigation and stability.
• Monitor patients receiving brief interventions and short-term therapy with structured follow-ups.
• Accept referrals from trained staff to support stable patients needing accountability and continuity of care.
• Guide patients to higher levels of care or external resources and ensure successful connections.
• Collaborate with Care Coordinators on scheduling, chart transfers, referrals, and enrollment documentation.
• Attend huddles or case reviews with the BHC to discuss updates and adjust coaching strategies.
