Job summary

Access Health Louisiana is seeking a BH Case Management Specialist to advocate for patients by coordinating care across primary care and behavioral health services. You will manage a caseload, identify and monitor referrals, and support clients through transitional periods of care.

The role requires strong communication skills, HIPAA knowledge and the ability to work with insurers to determine coverage and eligibility. Experience in behavioral health is essential.

Qualifications

• Master’s degree from an accredited school of social work, psychology, substance use or related human services profession.
• Three or more years of experience working in behavioral health.
• Knowledge of HIPAA laws related to mental health and substance misuse treatment.
• Knowledge of insurers requirements/coverage and ability to discuss them with BH patients.

Responsibilities

• Screen and assess clients; maintain confidentiality.
• Obtain referrals, prior authorizations and related information for services.
• Coordinate care between primary care and behavioral health.
• Provide substance abuse education to students, families and health care practitioners.
• Document client care and progress; adjust service plans as needed.
