Full-time, non-exempt position reporting to the Primary Care Health Home Director; experience in health coaching and collaboration with multidisciplinary teams is valued.

Qualifications

  • A bachelor’s degree in social work or a related human-services discipline.
  • Experience in health coaching or patient education is preferred.
  • Ability to work in a multidisciplinary primary care team.

  • Administer approved brief behavioral health, substance use, health-risk, and social-risk screens.
  • Provide patient education, health coaching, and follow-up on self-management goals.
  • Support navigation and reminders for brief interventions, short-term therapy, and referrals.
  • Refer concerns needing higher level care to a licensed clinician.
  • Provide PCHH contacts monthly to assist in care management and referrals.
  • Assist care team in high utilizers follow-up and care planning.
  • Provide behavioral health support to improve health status and manage chronic illnesses.
  • Assist with medication adherence and treatment plan adherence.

PCHH Behavioral Health Assistant

Full-Time Non- Exempt

The Behavioral Health Assistant (BHA) reports to the Primary Care Health Home Director. The BHA provides health and behavior coaching to patients with chronic health illnesses. The BHA is a part of the PCHH care team and provides direct and indirect services to people served in the primary care setting.

Responsibilities
  • Work cooperatively with MHHC’s providers and personnel to carry out the goals and objectives of MHHC, according to established policies and procedures.
  • Administering approved brief behavioral health, substance use, health-risk, and social-risk screens
  • under established protocols.
  • Providing patient education, health coaching, reinforcement of healthy behaviors, and follow-up on
  • self-management and care-plan goals.
  • Supporting navigation and reminders for brief interventions, short-term therapy, and referrals.
  • Referring concerns that require assessment, diagnosis, treatment, or a higher level of care to a licensed clinician.
  • Provide required Patient Centered Health Home (PCHH) contacts monthly in order to assist in care management, care coordination, health promotion, transition of care, referrals and patient/family support.
  • Assist care team in high utilizer follow-up and plan of care.
  • Behavioral health support to assist patients in improving health status and managing chronic illnesses.
  • Provide assistance with medication adherence, treatment plan adherence, self-management support, goal setting and/or facilitate group classes.
  • Provide screening, brief intervention/education, brief coaching and referrals for substance use disorders.
  • Focusing on managing a population of patients versus specialty care.
  • Provide educational materials, handouts and discussion of strategies individualized to meet patients’ needs. Participate in development and selection of patient education materials and services.
  • Function as an integral part of an integrated, multidisciplinary team including primary care providers, Medication Assisted Treatment (MAT) providers, behavioral health providers, psychiatrists, psychologists, community health workers, peer support workers, care managers and clinical nurses.
  • Attend and participate in staff meetings, huddles, department meetings and other meetings/trainings as assigned.
  • Others duties as assigned.

Education
  • A bachelor’s degree in social work or a related human-services discipline.
