Job summary

Intermountain Centers is hiring a Care Coordinator to manage members’ whole health using an integrated case management model. You will issue assessments, plan and coordinate services, and document in the Electronic Health Record, collaborating with a multidisciplinary team to support positive outcomes.

The role requires a behavioral health background, strong documentation, and the ability to navigate licensure supervision and reimbursement processes.

Qualifications

• Bachelor’s degree in a field related to behavioral health, social work, etc.
• Associates degree with 2 years related experience OR high school diploma with 4 years in behavioral health.

Responsibilities

• Observe members and document concerns per policy.
• Maintain member health records and treatment plans.
• Coordinate services and communicate with an interdisciplinary team.
• Develop individual service plans with time-specific actions.
• Support engagement and supervision requirements as needed.
• Follow HIPAA/privacy controls and agency policies.
