Job summary

Qualifications

• High School Diploma or GED required; Bachelor's degree preferred.
• Minimum 3 years of case management experience.
• Ability to work under stressful conditions and irregular hours.

Responsibilities

• Coordinate client care with therapists and care team.
• Arrange family visits and transportation.
• Maintain HIPAA-compliant documentation and billing in the EHR.
• Attend supervisory and team meetings and present case reports.
• Coordinate ISP and crisis management as needed.
