Job summary

Ole Health is seeking a Behavioral Health Navigator for its Integrated Behavioral Health (IBH) program in California. The role focuses on closing referral loops, improving access to behavioral health services, and acting as a key liaison for patients, families, and care teams.

The position emphasizes collaboration with IBH providers, medical staff, and community partners to enhance care coordination, timely appointments, and patient outcomes.

Qualifications

• Education: High School Diploma or equivalent; undergraduate social services degree preferred.
• Experience working with diverse populations and in community health settings preferred.
• BLS certification required.

Responsibilities

• Process and close the loop on internal and external behavioral health referrals.
• Support quality measure initiatives.
• Serve as point of contact, advocate, and informational resource for patients, family/caregiver(s), primary care team, payers, and community resources.
• Facilitate access to internal and external behavioral health providers and services.
• Contact scheduled initial IBH appointments to assess readiness for appointment.
• Minimize missed opportunities by managing the IBH schedules and patient wait list.
• Develop and maintain relationships with community partners to ensure robust collaboration.
• Participate in huddles, team meetings, and case conferencing as needed.
• Help facilitate warm handoffs at all sites.
• Collaborate with care coordinators to limit overlap and ensure closed-loop IBH referrals.
